multiple report updates to fix citation issues

The following changes were submitted to the "OSU P&T CV" and "OSU Comprehensive CV" reports.  These changes are mostly to reflect the split of the Journal Name/Publisher of Item field into two separate fields.

  • D.1.a – Service - Add “Brief Description of Committee's Key Accomplishments” to citation

  • D.1.b – Add “Brief Description of Committee's Key Accomplishments” to citation

  • D.1.c – Add “Brief Description of Committee's Key Accomplishments” to citation

  • D.2 – Add “Brief Description of Committee's Key Accomplishments” to citation

  • C.1.* - Update all publication citation to reflect split of Publisher and Journal Name into two fields.

  • B.1.C – NonCredit Courses and Workshops - Update citation with punctuation and to include the role of the person who’s report is being run.

  • B.3 – Peer Teaching Evaluations

    • a. update citation for credit events to include course pre and course num

    • a & b. Update citation to output the score.

These changes were also applied to the respective sections of the "Annual Activity" report.  However in this report, the Peer Teaching Evaluations section didn't exist, so it has been added with this request.
