Minor update requested to the Annual Activity report by Extension:

Section Collaborations and Partnerships.
Add Description tag
Add Contact Information output

Outputting the Contact Info field will benefit Extension AOM's during annual reviews enabling supervisors to see collaborator contact info and use if desired.
